I did a playthrough that got me InevitableDarkness_KilledOnHard but when I got to playful darkness I only got a +6 belt of perfection, not a +8 flawless belt of perfection, while that may just be a bug, I'm wondering if/where it'd spawn the +7 flawless cloak of resistance. IIRC the ring is in the act 2 cemetery, the belt is with playful ...

I just beat Playful Darkness on Unfair and didn't get the achievement. The title pretty much says it all. I dropped the difficulty down to core and I still didn't get the achievement.
