I call it Honest because the normal difficulty increase of Vanilla game just artificially raises enemy damage and lowers yours. None of that here! Here the enemies are just very very strong (and so are you, when you level up). Now every enemy has a chance to spawn.

They're very different, so it depends on what you like. Very hard is the normal game, but your guns don't do damage, while the enemy damage gets massively boosted. Other than that it's just like playing normally. Survival on the other hand tweaks things massively, so the game feels very different to play.
